    Sound Beginnings Infant Hearing Aid Program Information for Interested Clubs

    Now, the We Share Foundation is offering clubs in the United States the opportunity to distribute these hearing aids to qualified applicants—babies who are:

    • Age birth to 36 months and reside in the United States

    • Recently diagnosed with permanent hearing loss by a licensed audiologist

    • From a family whose income is below to 50% above the federal poverty level or family income is above current poverty guidelines, but special circumstances put the family in need of assistance

    • Not covered by public or private health insurance OR covered by an insurance policy that does not cover the cost of hearing aids

    This Sound Beginnings project is an opportunity to make a tremendous difference in the lives of children and their families. The We Share Foundation is providing the materials and tools you need to adopt this program, allowing you to focus on finding qualified applicants and making it easier to publicize this initiative in the community.

    Sound Beginnings Infant Hearing Aid Program Tool Kit
    We are happy to send ANY of these materials to your club via e-mail as a Word document so that you can customize them for your club's use. Contact brooke@quota.org. To view or print materials, click on the links below:

    • The Sound Beginnings Program Manual for Clubs*—Read through this manual and you will be ready to operate Sound Beginnings in your Quota club. This guide summarizes all of the steps you will need to take, procedures for identifying and screening babies, the application and reporting process, publicity tips, and more.

    • Sound Beginnings Program Overview*—An information sheet that you can share with your fellow club members.

    • Frequently Asked Questions*—A two-page sheet containing answers to common questions from club members and applicants.

    • Sample Letter to Service Providers*—Use this letter to reach out to service providers, who might be able to connect you to a baby who would qualify for and benefit from this program.

    • Information Brochure*—Add your club's contact information to this information brochure, and it's ready to distribute to families in need in your community.

    • Sample Press Release*—Customize and use this press release to gain valuable community publicity for your club and to attract possible new program applicants.

    • Application for Assistance, Applicant Agreement/Contract/Reporting Guidelines*—Add your club's contact information to this application, and it is ready to distribute to families interested in participating. Once this document is completed by the family, it is reviewed by your club; if approved by your club, the application is then forwarded to the We Share Foundation. The agreement/contract is signed by both the applicant family and a club representative.

    • Club Application*—The form your club completes and submits to Quota International's We Share Foundation along with an application for assistance your club has approved.

    • Family Report Forms*—We are providing two report forms that the family will complete and return to your club. (Your club will forward these reports to the We Share Foundation.) The first report is due to your club within four weeks after the baby's fitting, and the second report is due after six months of the fitting.

    • Club Report Forms*—Your club will be expected to complete two reports during the course of the project. The first is due within four weeks of the baby's hearing aid fitting and the second report is due at six months after the baby's fitting. A copy of the family report will accompany your reports.
